We opened our Forest Lake office in 1991 in downtown Forest Lake just north of the intersection of Lake St. (Highway 61) and Broadway (Highway 2). Our office is on the second floor. We have had a longstanding strong relationship with Forest Lake Allina clinic doctors as well as local surgeons from Summit and St. Croix Orthopaedics. Our therapists also see many patients from other physicians and we also gladly see patients on self referrals.
Our therapists all deliver excellent orthopedic rehabilitation, and we provide specialty therapy services including occupational health through The Work Center, hand therapy, women’s health, aquatics, and biofeedback.
Our Forest
Lake staff is very caring, friendly, and highly experienced. We have 5 highly skilled physical and occupational therapists and 1 physical therapist assistant. Jan Olson, PT, OT, CHT; Sheila Maassen, PT; Tammy Kasprowicz, PT; and Trixie McConnell, PTA, each have more than 10 years of clinical experience; they are joined by Andy Grupa, PT and Wendy Camp, OT. Jan Olson has been with OSI for over 20 years and was one of the first certified hand therapists (CHT) in Minnesota.

To get to our Forest Lake office:
From the South and West or North and West. Get on Interstate 35. From 35, exit on Broadway (Highway 2) and turn east. Take Broadway east to 61 (Lake St) and turn left (north) on 61. Go one block and you will see our building on the northwest corner of Lake and 2nd Avenue. Turn left (west) on 2nd Ave. at the Forest Lake Police Station and park in the parking lot on the west side of our building. You may be able to find on-street parking on Lake St. in front of our building. From the South and East or North and East. Take either Highway 97 or Highway 8 to Highway 61. From 8 go south on 61 and from 97, go north on 61. In both cases Highway 61 becomes Lake St. Our office is located at 146 N. Lake St., which is at the corner of Lake and 2nd Avenue. Off of Lake St., turn west on 2nd Avenue (at the Forest Lake Police Station) and park in the parking lot on the west side of our building.
You can take the elevator or stairs to second floor, and our office is right behind the elevators.
